TRICARE credentialing for therapists: certified vs network, East vs West

Therapists work with TRICARE through a regional contractor, Humana Military in the East or TriWest in the West, and have two levels of participation: certified non-network provider (authorized to treat and bill TRICARE clients, no contract, minimal paperwork) or full network provider (contracted, listed in the directory, negotiated rates). Payment follows the CMAC fee schedule, which generally aligns with Medicare rates.

If you practice anywhere near a base, TRICARE clients are already finding you. Most therapists assume the choice is binary, join the TRICARE network or turn those clients away, and the regional contractors barely advertise the certified middle option, which is less work than any commercial panel.

East or West: which contractor owns your state?

Two contractors split the country. Humana Military administers the East Region; TriWest Healthcare Alliance administers the West. The line moved on January 1, 2025, when six states (Arkansas, Illinois, Louisiana, Oklahoma, Texas, and Wisconsin) shifted from East to West. Everything about your TRICARE relationship (certification, network application, claims, provider portal) runs through your region's contractor, not through TRICARE centrally.

The 2025 shift is still biting therapists in the moved states, because old East contracts did not carry over. West Region network participation requires signing with TriWest, and until that happens, a therapist who held a Humana Military contract for years is an out-of-network provider to their existing TRICARE clients. If you're in one of the six moved states with an old East contract, verify your TriWest status before booking another TRICARE client.

What's the difference between certified and network?

Certification is authorization; network is a contract. Every provider who touches TRICARE money must first be TRICARE-certified (the contractors also say "TRICARE-authorized"): the contractor verifies your license and qualifications and approves you to treat beneficiaries. That alone makes you a non-network authorized provider: TRICARE clients can see you and TRICARE pays, with no contract, no directory listing, and no network obligations.

The network contract is the optional second step: you agree to the contractor's terms and negotiated rates, file claims per network rules, and in exchange you're listed in the directory and referrals flow to you, including TRICARE Prime patients, whose managed-care plan steers them to network providers. Prime is the catch for non-network providers: Prime patients seeing you without a referral pay steep point-of-service costs, so certified-only status realistically serves TRICARE Select families, not Prime.

Provider-type rules differ by license. TRICARE-certified marriage and family therapists sign a participation agreement and accept the allowable charge as payment in full even outside the network. Mental health counselors must meet TRICARE's own certified mental health counselor standard, which layers education and supervised-practice criteria on top of state licensure. Psychologists and LCSWs have the simplest authorized-provider path. Confirm your license type with your regional contractor before applying.

What does TRICARE pay?

TRICARE's ceiling is the CHAMPUS Maximum Allowable Charge (CMAC), and per TriWest's reimbursement methodology it generally aligns with Medicare's rates and rules for equivalent services, adjusted by locality and site of service. In practice, think "roughly Medicare" for a 90834 or 90837 in your area, and look up the exact figures in the CMAC search tool on health.mil, the same codes you'd check in the Medicare fee schedule if you're weighing both panels.

Network providers agree to the contracted rate, which can sit below CMAC as the price of directory placement and Prime referrals. Non-network participating providers are paid the allowable and accept it as payment in full. Where CMAC hasn't set a rate for a code, state prevailing rates apply, but the standard psychotherapy codes are all CMAC-priced.

Who is TRICARE actually worth it for?

Therapists within commuting distance of an installation, first: in markets like Norfolk, San Diego, Killeen, Fayetteville, Colorado Springs, and Tacoma, TRICARE can be the single largest payer in the market while panels of every other kind still close. Second, anyone already seeing a military spouse or veteran privately who'd rather not lose them to a PCS-driven budget squeeze. Telehealth counts: TRICARE covers it, and military families move constantly.

The certified-only route is the right first move for most solo therapists: it's the lowest-effort payer authorization in behavioral health, it makes the clients already calling you billable, and it generates the demand data that tells you whether a network contract is worth the credentialing file. STEP 3: APPLY (BY REGION) EAST (Humana Military):

  2. Certified: submit the certification application for your license type (MFTs sign the participation agreement; MHCs meet the TRICARE-certified mental health counselor criteria)
  3. Network: after certification, submit the join-the-network request and credentialing file (license, malpractice, CAQH-style history) WEST (TriWest):

  5. Same two levels; if you held a Humana East contract in a moved state, you must sign with TriWest. Verify your current status first.
  6. Register for the TriWest provider portal for claims and status

STEP 4: BEFORE YOUR FIRST TRICARE CLIENT
